A report on the death of five people in the Titan submersible blames design, maintenance, and inspection flaws for its failure. “It all came back to Mr. Rush,” the head of the investigation told WIRED.

The 300-page report calls the implosion a "preventable tragedy" and reveals criminal charges could have followed if CEO Stockton Rush had survived.

The report also concluded there was evidence of ‘potential criminal offences’ in the case of Mr Rush, saying he had ‘exhibited negligence that contributed to the deaths of four individuals’ and may have been accused of ‘misconduct or neglect of ship officers’ had he survived the incident.
